I get it. By "comes" he means cums, which is slang for ejaculation of semen. The indication is that he masturbates only once a year, so ejaculates a lot of semen. I don't mind very grosse humor, but it didn't really seem funny to me.

Poor Poor Mrs Santa how does she manage the fact that santa has an erection problem but has a large sack and comes once a year and not with her.
